Lifetime Achievement Award


  • The Louisiana Governor’s Safety and Health Conference’s Lifetime Achievement in Safety and Health Award will be given to an individual or individuals who have made an outstanding contribution to workplace health and safety or to improving community safety through significant personal leadership, initiative, personal commitment and/or volunteer activity above and beyond the norm required by his/her employment. This award is not necessarily given annually.

    Criteria for judging nominees include:

    • Evidence of the nominee’s contribution to promoting safety, health and emergency response in the workplace and/or the community;
    • Evidence of the nominee’s  service  as a mentor to others in the field of safety, health and emergency response;
    • Evidence that the nominee is courageous about speaking out on important issues in the field of safety, health and emergency response via offering constructive criticism and solutions when appropriate; and,
    • Evidence that the nominee has been highly committed to safety, health and emergency response throughout his/her career.

    Nomination packets must include:

    • Nomination form;
    • 200-500 word essay describing  why the statement describing why the award should be given to the nominee;
    • Three letters of recommendation; and,
    • No more than three additional supporting documents such as articles, etc.

     

    The recipient(s) of the 2010 Louisiana Governor's Conference Lifetime Achievement in Safety, Health and Emergency Response Award will be recognized at a special dinner in conjunction with the Louisiana Governor’s Safety and Health Conference on Tuesday, October 26, 2010, at the Hilton Capitol Center's Heidelberg Room. 

    All nominations are confidential and will be judged by a select panel. Nominations due by to September 21, 2010.
